GESEA MIGS Diploma: Surgical Competence Meets Rigorous Evaluation

For gynaecological surgeons committed to mastering minimally invasive techniques, the GESEA MIGS Diploma represents one of the most advanced certifications available in the field.

But unlike theoretical exams or psychomotor evaluations, the MIGS Diploma goes a step further — it assesses real surgical competence, in the operating room, on real procedures, through expert-reviewed videos.

A Diploma Based on Practice, Not Theory

The MIGS Diploma is awarded only after a surgeon has demonstrated competence in both laparoscopy and hysteroscopy, through documented clinical practice and expert evaluation of surgical video cases.

It is not a one-day test — it is the culmination of a rigorous, structured process that may span up to five years of surgical activity.

How to Obtain the GESEA MIGS Diploma

1. Meet the Prerequisites

Before applying, you must have:

  • A valid GESEA MIGS Certificate

  • A documented surgical curriculum, with supporting documentation, sent to info@esge.orgAttachment.tiff

2. Schedule Your Procedures

You must schedule and perform five separate procedures — using new, not previously recorded material.

Once the first procedure is scheduled, you are officially enrolled in the MIGS Diploma pathway.

Each video must be submitted within 48 hours of the surgery, along with the anaesthesiology report.

All submissions are reviewed by independent international experts through the GESEA Diploma Video submission platform.

What Will Be Evaluated?

Laparoscopic Hysterectomy

Candidates must submit a total laparoscopic hysterectomy performed independently. The evaluation focuses on four critical steps:

  1. Dissection of the broad ligament and infundibulopelvic ligament

  2. Bladder dissection

  3. Colpotomy

  4. Laparoscopic vaginal suture

These steps are assessed using the H-OSATS scale, an objective tool that evaluates surgical precision, tissue handling, and operative flow.

➡️ A reference article titled “Surgical steps of total laparoscopic hysterectomy” is available in Facts, Views & Vision in ObGyn, the ESGE’s official journal.

Hysteroscopic Procedures

Applicants must submit four hysteroscopies (all performed as first surgeon):

  • 2 diagnostic cases:

    • One standard case of any kind

    • One in a menopausal or nulliparous patient

  • 2 operative cases:

    • One polypectomy (polyp ≥1.5 cm)

    • One myomectomy

The evaluation focuses on hysteroscopic technique, diagnostic reasoning, anatomical detail, and use of adjunct imaging where relevant.
Particular attention is given to:

  • Vaginoscopic, non-traumatic entry

  • Navigation and fluid control

  • Panoramic and close-up views

  • Endometrial evaluation and diagnosis

  • Structured description of pathology

  • Differential diagnosis when applicable

A Pathway of Excellence

The GESEA MIGS Diploma isn’t simply a title. It’s a demonstration of surgical maturity, validated through peer review and high-level scrutiny.

It reflects not only skill, but also judgement, anatomical understanding, and commitment to safe, evidence-based surgery.

For surgeons ready to take the next step in their training — and to be recognised across Europe as fully competent MIGS professionals — this diploma offers a meaningful, respected, and transformative goal.
