What is GESEA? Understanding the Structured Pathway to the MIGS Diploma

The Gynaecological Endoscopic Surgical Education and Assessment (GESEA) programme is a structured educational pathway designed for gynaecological and reproductive endoscopy. Its objective is clear: to train and certify knowledge and practical skills before surgical competence is exercised in the operating room.

Built in accordance with the European–American Joint Recommendation, GESEA follows the principle that every hospital teaching endoscopic surgery should provide access to a structured dry lab environment. Simulation-based training is scientifically proven to improve surgical performance, enhance patient safety, and optimise the learning curve. GESEA integrates this philosophy into a progressive, validated system of certification.

The programme spans three levels of increasing proficiency. At the GESEA Training Center in Naples, the focus is on the MIGS (Minimal Invasive Gynaecological Surgeon) pathway, offering a complete route from Level 1 to the MIGS Diploma.

The GESEA MIGS Pathway allows you to obtain the official ESGE Minimal Invasive Gynaecological Surgeon (MIGS) Diploma through a structured three-step progression.

Step 1 – GESEA Level 1 Certificate

Your entry point into the Operating Room.

Level 1 certifies general endoscopic knowledge and the acquisition of basic psychomotor skills required before beginning supervised surgical training.

To obtain Level 1, candidates must:

  • Complete the GESEA Level 1 e-learning on ESGE Academy

  • Receive the Level 1 Certificate of Completion

  • Train using validated GESEA training tools

  • Pass the certification session consisting of:

    • TESTT1 (theoretical exam – 50 MCQs)

    • LASTT1

    • SUTT1

    • HYSTT1

It is important to note that the TESTT1 examination includes additional questions beyond the preparatory tutorials, ensuring a comprehensive assessment of expected knowledge levels.

Step 2 – GESEA Level 2 MIGS Certificate

Start performing basic endoscopic procedures.

Level 2 represents the transition from foundational skills to advanced psychomotor competence required for performing basic endoscopic procedures in the operating room.

Prerequisites include:

  • Completion of the GESEA Level 1 Certificate

  • Completion of the Level 2 MIGS e-learning on ESGE Academy

Candidates may apply for a bypass only if they meet strict criteria, including documented surgical experience and proof of advanced suturing training, although Level 1 and Level 2 e-learning remain mandatory.

The Level 2 certification session includes:

  • TESTT2 (50 MCQs covering instrumentation, OR organisation, anatomy, techniques, complication management)

  • LASTT2

  • SUTT2

  • HYSTT2

As with Level 1, TESTT2 includes additional knowledge expectations beyond the tutorial content.

Step 3 – GESEA MIGS Diploma

Validate your surgical competence.

Once the MIGS Certificate is obtained and sufficient surgical experience has been acquired, candidates may apply for the GESEA MIGS Diploma, issued by the ESGE.

The MIGS Diploma is a video-based, expert-reviewed certification, evaluating real surgical performance in selected laparoscopic and hysteroscopic procedures.

To apply, candidates must:

  • Hold the MIGS Certificate

  • Be an ESGE member

  • Provide documented surgical clinical experience (up to 5 years)

  • Schedule five new procedures (previous recordings cannot be used)

  • Submit each surgical video within 48 hours via the official GESEA platform

  • Include the anaesthesiologist’s report with each submission

All submitted procedures are independently reviewed and rated by international experts. This process ensures that the MIGS Diploma represents not only theoretical and simulated competence, but validated real-world surgical proficiency.

An additional, optional step after Level 2 is obtaining Robotics certification and diploma, further expanding psychomotor and technological expertise.

The MIGS Pathway at GESEA Naples

At the GESEA Training Center of the University of Naples Federico II, the available structured pathway is the MIGS track. Candidates can complete Level 1, Level 2 MIGS certification, and prepare for the final MIGS Diploma within an accredited and internationally recognised training environment.

Through validated training tools, structured assessment, and expert mentorship, GESEA Naples supports surgeons in progressing from foundational psychomotor training to advanced, expert-reviewed surgical competence.

GESEA is not simply about passing exams.

It is about building measurable, documented, and internationally recognised competence in minimally invasive gynaecological surgery.
